The longest word to appear in an English language dictionary is

Pneumonoultramicroscopicsilicovolcanoconiosis 

which is a lung disease caused by the inhalation of very fine silica dust, causing inflammation in the lungs.  It has 45 letters.  Spell-check, however, does not seem to like it.
